Grandmother and Toddler Caught in Tragic Flowell Murder-Suicide

FLOWELL, UT – The Millard County Sheriff’s Office has identified the three individuals found dead in a heartbreaking incident on July 24, 2025, in the small, rural community of Flowell, located west of I-15 between Fillmore and Delta. The victims are 28-year-old Jay Dean Stevens, his 67-year-old grandmother, Colleen Foutz Halsey, and his 23-month-old daughter, Harper Helena Stevens.
According to a statement from the Millard County Sheriff’s Office, deputies responded to a report of multiple deaths at approximately 8:30 p.m. on Thursday, July 24, at a residence in Flowell. Upon arrival, they discovered the bodies of Stevens, Halsey, and young Harper. Both adults died of apparent gunshot wounds, and a firearm consistent with the wounds was recovered at the scene. Investigators have determined that Jay Stevens is believed to have shot and killed his grandmother, Colleen Halsey, before dying by a self-inflicted gunshot wound, pointing to a murder-suicide.
The cause of death for Harper Helena Stevens remains undetermined pending autopsy results. Authorities have not yet clarified whether her death is related to the gunfire or another cause, and they are awaiting further forensic analysis to provide answers.
“This incident rises to every level of tragedy,” the sheriff’s office stated. “We are brokenhearted for family members who mourn these losses as they yet prepare for memorial services. We ask all to respect their privacy and feelings.” The office also acknowledged the speculation surrounding the incident and requested patience from the public, saying, “We are aware of the speculation going on about much of the incident and ask that we be allowed to complete the investigation. As we do so, and further information becomes available, we will provide further updates.”
No motive for the incident has been disclosed, and officials have not released details about the events leading up to the tragedy. The investigation remains ongoing as authorities work to piece together the circumstances of this devastating event. The sheriff’s office has confirmed that there is no ongoing threat to the public and no additional suspects are being sought.
The close-knit Flowell community, described by residents as welcoming and tight-knit, is grappling with the profound loss. One local, Lesha Swallow, who knew two of the victims, described Colleen Halsey as “the sweetest person” who would stop to chat during walks. Another resident, Brett Mathis, noted the community’s shock, especially as the tragedy followed a joyful Pioneer Day celebration hosted by the local ward of The Church of Jesus Christ of Latter-day Saints. “It’s been difficult for the community to go from a positive atmosphere of celebrating to an atmosphere of mourning the loss of life,” Mathis shared.
Jay Dean Stevens is remembered by his family as a lover of the outdoors, earning the nickname “Jay Bird” for his fondness for nature and birds. His obituary highlights his passion for camping, working on Jeeps with his father, and cherishing time with his daughter, Harper, whom he adored. A combined funeral service for Colleen Halsey and Harper Helena Stevens is scheduled for Friday, August 1, 2025, at the Flowell Ward of The Church of Jesus Christ of Latter-day Saints in Fillmore, with a viewing from 9:30 to 10:30 a.m. and services beginning at 11 a.m.
